Call the office and Devon will answer any questions you might have.  She answers phones from 9-3.  We drive by appointment only.  She can walk you through the Arizona Motor Vehicle process required to return to driving.  If you have had your license suspended or revoked, she can assist you with how to get started again.  She has the appropriate forms required by MVD to accomplish this including the Medical Exam Form, and  the Vision Form.  If you have had seizures, you must be seizure free for 3 months before re-evaluation.
