Why do I need this test?
Arizona Statute states that if you have had a change in medical condition you must report this to Motor Vehicle. This is Motor Vehicle’s way of keeping us all safe on the roads. Your doctor and therapists are following the statute. If you were to have an accident without going through this procedure, there could be repercussions since you have had a major change in your medical condition.
Does my insurance cover this cost?
Unfortunately no. They do not see driving as a medical necessity, therefore they don’t cover it.
How much will it cost and how long will it take?
If you have workman’s compensation they might pay or if you have another funding source, you should check with them first. Private pay is a reduced amount of $250 or $400 depending on the type of evaluation. It takes about 2-4 hours. Training is $100/hour. Training for hand controls averages about five hours and for a left foot gas pedal about two hours in addition to the evaluation session. We must also visually inspect your vehicle once it is adapted.

How do I make an appointment?

What do I need to bring?
You will drive Lynn’s evaluation vehicle. But you may need to have someone drive your car to her office at the time of the appointment so she can see your vehicle if it needs adapting. Bring your glasses to drive if you wear them and of course your drivers license. Lynn does not accept credit cards. Payment is by check or cash.
