FAQ

Why do I need this test?

Arizona Regulation states that if you have had a change in medical condition you must report this to the Motor Vehicle Department.  This is Motor Vehicle’s way of keeping us all safe on the roads.  Your doctor and therapists are following the regulation. This is the Motor Vehicle Department requirement.

Does my insurance cover this cost?

Unfortunately no.  They do not see driving as medically necessary, therefore they do not cover it.

How much will it cost and how long will it take?

Costs can vary depending on the type of evaluation that needs to be completed. Please contact the office to check on the price of your specific evaluation. The evaluation itself generally takes 2 to 4 hours depending on the type of evaluation. The amount of training needed varies from case to case. We must also visually inspect your vehicle once it is adapted and there is an additional charge for this.

There are a few government programs that are able to help cover the costs in certain situations. Please contact our office and speak with one of our staff members if you would like to see if you qualify for one of these programs.

How do I make an appointment?

Call the office at 602-840-2323 for the initial phone consult.

What do I need to bring?

You will drive our evaluation vehicle.  But you may need to have someone drive your vehicle to our office at the time of the appointment so the evaluator can visually inspect it. Also bring your glasses to drive if you wear them and of course your Driver’s License.  We will also require a current list of medications.
